Minimum Criteria Needed for Authorizing Pediatric Subacute Care
  1. Patient’s condition has stabilized to the point that acute care is NOT medically necessary; and

  2. Patient’s condition warrants 24 hour nursing care by a registered nurse; and

  3. Any one of the following four items:

    1. A tracheostomy with dependence on mechanical ventilation for a minimum of six hours each day; or

    2. Dependence on tracheostomy care requiring suctioning at least every six hours, and room air mist, or oxygen needed and dependence on one of the four treatment procedures listed in Section 4(b) through (e); or

    3.  Dependence on total parental nutrition or other intravenous nutritional support requiring and dependence on one of the five treatment procedures listed in Section 4(a) through (e); or

    4. Dependence on skilled nursing care in the administration of any three treatment procedures listed in Section 4(a) through (e).
  1. Treatment Procedures:

    1. Intermittent suctioning at least every eight hours, and room air mist or oxygen as needed;

    2. Continuous intravenous therapy, including administration of therapeutic agents as necessary for hydration or of intravenous pharmaceuticals; or intravenous pharmaceutical administration of more than one agent, via peripheral or central line, without continuous infusion;

    3. Peritoneal dialysis treatments requiring at least four exchanges every 24 hours;

    4. Tube feeding, naso-gastric or gastrostomy tube;

    5. Other daily medical technologies required continuously which in the opinion of the attending physician and the Medi-Cal consultant require the services of a professional nurse. 
Level of Care

We use the following levels to identify service needs.

Levels of Services
*Per diems reflect an all-inclusive rate, except for doctor's fees, special bed, or the fees for a specialized medical device, such as a V.A.C. machine.

Level 1 Intensive Care: Patients are medically fragile but stable, requiring intense, technical care.

  1. Ventilator dependency - intermittent or continuous
  2. Patients who require RN or RT observation and/or treatment at least every two (2) hours
Level 2 Medical/Surgical with or without Rehabilitation
  1. Tracheotomy Care/ Weaning
  2. Intermittent I.V. Therapy
  3. Continuous peripheral I.V. therapy with or without medication
  4. Continuous subcutaneous infusion
  5. Continuous central line or port therapy with or without medication
  6. Chest percussion or postural drainage
  7. Gastrostomy or nasal-gastric tube feeding
  8. Enterostomal or fistula care (including patient and family teaching)
  9. Intensive wound care: clean or sterile technique at least 3 times/day
  10. Post-op care
  11. Apnea monitoring
  12. Suctioning-oral, pharyngeal, tracheal at least every four (4) hours
  13. Intrathecal baclofen pump management
  14. TPN Therapy
  15. Isolation
  16. Coma or semi-comatose status
  17. Terminal illness care/Pain management
  18. Neurological assessment and stimulation
  19. Respiratory Therapy (i.e. IPV)
  20. Cardiac Rehabilitation
Level 3 Rehabilitation
  1. Physical Therapy
  2. Occupational Therapy
  3. Speech Therapy
  4. Splinting
  5. Cast Care
  6. Goal D/C to acute rehab
  7. Tone Management
  8. Discharge Therapy
