Case Management

We provide case management to each and every child to identify care needs and create a plan of care to align with the child/family's goal, to ensure that they receive the optimal benefits from the care and treatment, utilize and or coordinate with the local or state agencies for programs/sources /referrals, such as the Early Intervention Programs, Santa Clara County Office of Education, Regional Center, California Children's Services, and Child and Adult Protective Services. 

The Case Manager coordinates discharge planning and assists in educationally, emotionally, and functionally preparing the child and their family. The goal of case management is to assist the child and family in becoming the case manager so that they can advocate on behalf of themselves and/or their child. Case management at SASCH strives to promote and develop ties between the child/family and the referral network.

Social Work

Social worker provides psychosocial support, counseling including crisis intervention, individual, group and family counseling and community referrals.

Family Support Group (FSG)

FSG provides peer support, advocacy and information to the families of the children at SASCH. Knowing that they are not alone greatly help the families cope. FSG meetings are held once a month, facilitated by the family members themselves. They plan activities to get to know each other, share information, and support each other.
